Cancellation & No Shows

24 hours cancellations notice is required.

Failure to cancel without advanced notice, client will be billed 100% of the total time booked.

If no show, cancellation or frequent rescheduling occurs a client will be dropped and unable to book future appointments with LASER.EHR.

 

Privacy

All of clients personal information and health history is legally protected under HIPAA.

Late Arrivals

Please arrive on time for your appointment. If you do not make it on time I will not be able to accommodate you for total session time booked. You will still be charged full amount of the session time booked.

 

Client Health History Assessment

All clients must complete a health history assessment form before your first session.

Please arrive 5-10 minutes early to give yourself enough time to complete it.

Payment Is Due At The Time of Treatment

All major credit cards are accepted.

Under 18 Clients

Clients under 18 years old must be accompanied by a parent or legal guardian.

 

Covid-19 Safety Precautions

To ensure our clients and technician are kept safe during the Covid-19 pandemic, you will be asked to keep a mask on at all times. Use hand sanitizer or wash your hands upon arrival into the office. Once in the office your temperature will be checked upon arrival.

Exception: If the area being worked on is where mask coverage is then client can remove mask until completion.
